Anatomically, the woman’s breast consists of a layer of subcutaneous fatty tissue as well as fatty, glandular and stromal tissues. From the inside, the breast is supported by Cooper’s ligaments playing the role of a framework. The shape and type of the breast depend on the state of each of its components and the size depends on the amount of fatty tissue. Therefore, an alternative to breast endoprosthetics is lipofilling, i.e. the grafting of one’s own fatty tissue.

What you need to consult before enlarging your breasts?

There are many clinics in Kyiv that perform breast augmentation surgeries, but not each of them is reliable. Before going to a consultation, you should check the reputation of the establishment and the qualification of its specialists.

Together with the doctor, the woman will decide on the brand, shape and volume of the implants, as well as their position in the body and the section technique. Patients often dream of radical changes in the size of breast glands, but an experienced surgeon will always try to persuade the woman to have more natural volumes that will not make the body look disproportionate and will not overload the backbone.

Types of breast implants

The alteration of the breast size is usually achieved by silicone implants chosen by the following criteria:

  • volume;
  • shape;
  • filling;
  • surface type.

The weight of the implant is larger than its volume. Thus, a woman that wants to have size 4 breasts will need to get used to their new weight – they will be around 1 kg heavier each.

In terms of the shape, implants are round and tear-shaped. They differ by the point of maximum section: it lies in the middle of round implants and in the lower third of tear-shaped ones. The latter type is also called anatomical because it is closer to the natural shape.

An implant can be filled with:

  • biogel;
  • physiological solution;
  • silicone gel.

Biogel contains organic elements, and, if the cover is damaged, the filler disintegrates into natural components in the body tissues. Silicone implants do not get deformed even in case of a rupture. The physiological solution filler is not particularly durable – these implants need to be replaced with time. But a much smaller cut is needed to insert them, and the recovery period is much simpler.

There are breast implants with a textured, polyurethane and smooth cover. The surgeon will determine the necessary surface type depending on the desired size and shape.

Indications to manipulation

Indications to mammoplasty include:

  1. Objectively small breasts.
  2. Woman’s desire to enlarge the breast size.
  3. Asymmetry.
  4. Alteration or lowering of breast glands.
  5. Negative consequences of a prior plastic surgery.
  6. Woman’s occupation demanding an alteration of the breast size.
  7. Condition after breast removal or treatment.

Examination before a breast augmentation surgery

The patient should have the following tests taken before the manipulation:

  • Ultrasound;
  • mammography;
  • ECG;
  • general and biochemical blood test;
  • coagulogram;
  • examination for venereal diseases.

Positioning of implants during the breast augmentation surgery

The location of implants is determined by the volume of the cover tissues and whether the woman intends to breastfeed in the future. Locations may be as follows:

  • subglandular;
  • subfascial;
  • submuscular;
  • biplanar.

In the subglandular and subfascial position, the implant is inserted under the gland, between the major pectoral muscle and the gland in the former case and between the greater pectoral muscle and its fascia in the latter case. In the submuscular location, the implant is completely inserted into the muscle. The biplanar location involves the positioning of the implant in a “double pocket” – under the glandular tissue at the bottom and between the muscles at the top.

Choosing the place to place the implant

Surgical access can be ensured through:

  • the crease under the breast gland — inframammary;
  • a cut around the nipple areola — periareolar;
  • the axillary cavity region — transaxillary;
  • the navel — transumbilical;
  • abdominal front line — transabdominal.

The first option is the most widely used. The section length is up to 4 cm. In this type of breast augmentation, the implant does not hinder further lactation

The cut around the nipple is almost invisible in the future, but it may affect breastfeeding.

The surgical access through the axillary cavity combines the inconspicuousness of the scar and does not affect lactation, but it is the most traumatic.

Only implants with physiological solution are inserted through the navel, and this type is sometimes used when mammoplasty is combined with other surgical manipulations.

Contraindications

Breast augmentation is not recommended in the following cases:

  • age under 18 years;
  • oncological diseases;
  • mastopathy;
  • worsening of chronic or infectious diseases;
  • blood coagulability disorders;
  • diabetes mellitus;
  • breastfeeding period;
  • severe internal diseases.

Psychological consultation before breast augmentation

Breast augmentation should not be a spontaneous decision. From the psychological point of view, surgical intervention is recommended if a woman:

  • feels constrained and unconfident;
  • chooses poses and gestures trying to conceal the breasts;
  • limits herself to closed clothes;
  • avoids socialization;
  • changes the usual lifestyle;
  • experiences psychological difficulties in the sexual life.

In these cases, breast augmentation becomes a salvation necessary to start living a full life.

What is a breast augmentation procedure?

Right before the surgical intervention, the surgeon marks future positions of implants on the patient’s body. The breast augmentation operation is performed at the inpatient department. General anesthesia is done. The doctor obtains the selected type of surgical access using a scalpel. If necessary, endoscopic equipment is used to ensure a complete view of the operation site.

After the preparatory stage, the surgeon inserts the implant according to the marks and prevents bleeding at the same time. The arrest of bleeding from small vessels of the surgical wound minimizes the risk of mammoplasty-related complications in the future.

When the formation of a “new” breast is over, the surgeon applies stripes of special stretching sterile plaster and bandages the operation site with elastic bandage. After this, special compression garment is put on the patient.

Duration of breast augmentation

The operation lasts for up to 2 hours. However, if a patient wishes to radically change the breast size, several operations may be required to gradually enlarge the volume of breast glands.

What is the recovery time for breast augmentation?

The breasts take on the new shape immediately after the operation, but the final result will be visible only after 4–8 weeks.

The patient is allowed to get out of bed on the second day after the intervention. First there will be swelling and discomfort. Pain is relieved with drugs.

Drainage is removed during the first days. Sutures are taken out on day 7–10 or they get reabsorbed depending on their type.

During the first month after the operation, the patient will wear compression garment that is supposed to retain the form and prevent the displacement of implants. During this period, it is also recommended to avoid physical exercise, going to the sauna, beach or solarium and it is prohibited to raise the arms. Sleeping on the stomach is allowed starting from the second month of rehabilitation.

Complete recovery after the operation takes around 2 months.

Result of breast augmentation

The breast augmentation operation will make the patient’s breasts look beautiful and fit and will raise her self-appraisal and life quality.

How long the effect after the operation will last largely depends on the choice of implant. The result is often lifelong, but in some cases soft tissues will need correction after 10–15 years or the implant will need to be replaced in case of its damage.
